Butler University College of Pharmacy and Health Sciences is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. Sessions in this homestudy learning activity are knowledge-based activities. Each participant in this homestudy program must submit a fully completed learning assessment and evaluation form in order to receive the credit specified for the UAN number. The total number of available CPE contact hours is 1.0 hour, or .10 CEUs. CPE statements of credit will be uploaded directly to ACPE within 7-14 days of receipt of the learning assessment and evaluation forms. UAN# is: 0017-0000-15-019-H04P. This homestudy program began on October 30, 2015 and expires October 28, 2017. Late assessments postmarked after 10/28/17 will not be scored or given credit.
